
Application
and Enrollment Process
Each child must have a completed application, child health information,
developmental health history, and registration fee before your child
can attend. You must also complete a Parent Agreement that indicates
your receipt of the Parent Manual and your understanding of its
contents. The Permission of Medication form is needed only if medicine
must be administered during EESP hours. A doctor’s note must
accompany this form.
EESP
can have up to 70 children in attendance at one time. Enrollment
is on a first come/ first served basis, however, once a child is
enrolled in EESP, the parent will have first priority for successive
semesters. A waiting list will be maintained after the limited enrollment
has been met and those parents will be contacted when a slot opens.
Payment
Policy
A $25 non-refundable registration fee per child is due at the time
of registration. Cash or check payments may be made. When making
check payments, please make checks payable to MTSU EESP. Please
write your child’s name on the FOR line at the bottom of the
check. You will receive a receipt for each payment.
There
will be a $20 service charge on all returned checks. After 2 returned
checks, only cash payments will be accepted.
Fees
are paid in advance on the first day of the week your child attends,
unless previous arrangements are made with the Director. Parents
are encouraged to pay on time or to prepay to avoid additional fees.
There will be a $5 late charge for accounts that are one week overdue.
If the account is three weeks overdue, your child(ren) will not
be able to attend the program until the account is paid in full
and the two weeks attendance policy (page 8) will also be applied.
If
you choose to leave EESP before the school year is over, you will
be mailed a refund for any pre-paid fees. Refund requests will be
submitted to the Business Office on the 15th of the month.
Billing
Procedure
All children must be scheduled for their time in EESP
and will be billed for those days whether the child attends or not.
Four (4) or more days are considered a week. EESP must have one
weeks notice if your child’s schedule will be changing. Schedules
may not be changed more than one time in a 4 week period.
EESP
is not a drop in facility. Therefore, children may
not come on an unscheduled day. Parents may add
days within a single week, if needed, and will be charged the daily
rate (4 day rule does not apply) in addition to the scheduled amount.
For example, if your child is scheduled for 3 days and you add 2
days to a given week, the billed amount will be $27 plus $18 for
the extra days.
EESP
is not required to hold your child’s spot in our program.
In the event that your child is absent, without payment,
for a period of two weeks, he/she will be withdrawn from the program.
In order to re-enter your child into EESP, you must pay a second
registration fee of $20. If your child is not able
to attend for a period of time greater than the two weeks allotted,
and you do not wish to continue payment for your child’s scheduled
time, you may choose to pay for 1 day per week to avoid the withdrawal
and re-enrollment process.
How
Payments are Processed
One person will be named as being responsible for a child’s
bill. If a parent is a student or employee of MTSU their EESP bill
be added to their preexisting account. If a parent is not affiliated
with MTSU, an account will be set up using the parent’s name
and address. All payments will be posted to that account and statements
will be mailed to that parent’s address. Any account 30 days
overdue will be transferred to MTSU as delinquent and sent to collections.
Snow
Days and Holidays
All children must be scheduled for holidays. Sign up sheets will
be placed on the front desk prior to the holiday. At least 10 children
need to be signed up for each day for EESP to be open. Parents must
cancel at least 24 hours before the scheduled day or they will still
be charged.
EESP
will be open at 7:45 a.m. until 6:00 p.m. for Rutherford County
bad weather days. If by 10:00 a.m., five (5) children have not confirmed
attendance EESP will close and those parents of children here will
be notified. A snack will be provided but parents need to provide
their child with a nutritious meal and beverage each day.
If
MTSU closes early, parents will be called and EESP will close.
Health
Policy
The Evening Extended School Program cannot provide for sick children.
Please, DO NOT send your child to the program if
they are ill. We are concerned for the health and welfare of each
child; therefore, we require that your child be picked up as soon
as possible in the event he/she becomes ill while in the program.
The “No Nit Policy” on lice for Murfreesboro City Schools
and Rutherford County Schools also applies to EESP. If your child
has been found to have bugs or nits they will have to be picked
up from the program. They will not be allowed to return until you
present the Director with proof that they have been treated and
the child has been rechecked and found to be clear of all nits.
Sick Policy
Children will not be allowed to attend EESP (or parents will be
called) if the child…
- …has
a fever or had a fever within the past 24 hours
- …has
vomited 2 times in one 24 hour period
- …has
had diarrhea 3 times in one day
Medication Policy
EESP will not dispense medications unless a Medication
Form (signed by a physician) is on file. Medications will only
be given if the physician states that it is medically necessary
for the child to have medication during EESP hours. Please note
that EESP cannot store medications which may need refrigeration.
Medications must be in its original prescription bottle and labeled
with:

Special
Needs Children
If your child has any health problems or conditions that may require
special care, you must provide a physician’s statement which
identifies the condition and which gives the physician’s special
instructions for the child’s care. You must also complete
the EESP Permission of Medication Form (if
needed) and obtain appropriate signatures.
Child
Abuse and Neglect Policy
Child abuse is the repeated mistreatment or neglect by parent(s)
or others resulting in injury or harm to a child. Child abuse/neglect
may be physical, verbal, emotional, or sexual. Child abuse/neglect
can result in severe emotional and /or physical handicaps, anti-social
behavior, and even death.
EESP STAFF IS REQUIRED BY LAW TO REPORT SUSPECTED ABUSE
AND/OR NEGLECT OF A CHILD.
Discipline
Policy
Children enrolled in the Evening Extended School Program are expected
to exhibit behavior which does not disrupt or interfere with the
school climate or the learning process and social interaction of
other children. EESP staff will enforce school rules throughout
EESP. Parents will be notified in writing when a major discipline
problem occurs with a Discipline Form. After three Discipline Forms,
your child may be suspended from the program for a period of 1-3
days. If there is another offense after returning from suspension,
you will be required to withdraw your child from the program.
An
exception to the above is when a major incident (deemed so by the
Director and/or Campus School Principal) occurs. This will result
in immediate suspension.
Parents
who demonstrate abusive language and/or inappropriate behavior or
disturbances to children, staff, or other adults will be required
to withdraw their children from EESP.
Procedure
for Resolving Parent Concerns
Parents will be requested to contact EESP Director (not Campus School
Principal or EESP teachers) immediately about any concerns they
have about EESP. The Director will respond to the concern using
EESP policies and/or procedures. If the concern is not resolved
through the problem solving techniques between the parent and the
Director, the parent will then be asked to complete a Parent Problem
Resolution Form. The forms will be submitted to the Director of
Administrative Service and the Vice President of Business and Finance.
They will work with the parties involved (and other staff as necessary)
to resolve parents’ concerns.
