Pre-conference workshop registration and payment due by March 19: $50 (no pre-conference workshop registrations will be accepted after March 19, 2004)
Regular registration and payment due from March 20-April 2: $110
Late and on-site registration: $150 (all registrations and payments received after April 2)
Hotel reservations should be made directly through the hotel by March 4.

Cancellation/Refund Policy

All cancellations must be submitted in writing to the address on this form. Cancellations postmarked by March 19, 2004, will receive a full refund.

No refunds after March 19, 2004.

Sign-up for regular workshops will be on a first-come, first-served basis at the conference registration desk:
