Checklist
to be used in Completion of the MTSU Instructional Leadership (Administrative
Licensure Program) Application
The following
materials and support documents will be collected during (or before) the end of
the first semester of the program. These
materials will be part of the requirements for FOED 6020. Incomplete admission may result in suspension
from the program until all materials have been submitted.
Required Support Materials for Master’s or Ed.S. Programs
These materials will be collected during the first class of the
program:
1.
A copy of your current teaching license or a copy of the MAT or
GRE Score
2.
A copy of your most recent performance evaluation.
3.
At least one letter of recommendation from your principal or other
supervisor that supports your candidacy.
The letter(s) should address your teaching skills and work with
students, leadership potential, or other evidence supporting for your entry
into the program.
4.
An interview form completed by a designee of your school system.
5.
An interview completed by a designee of the Leadership Program at
MTSU (will be completed during the FOED 6020 class)
These materials will be submitted to the
Graduate College:
1.
A copy of your current teaching license or a copy of the MAT or
GRE Score
2.
All transcripts from undergraduate colleges or universities – must
be official transcripts
Optional
Support Materials (Not
Required but May be Used to Support Candidacy)
1.
Additional support of teaching excellence – commendations, awards,
etc.
2.
Additional letters of support for your candidacy from fellow
teachers or previous administrators or supervisors.
3.
Evidence of professional development – certificates or other
documentation of training
4.
Student data or work samples that show innovation in teaching ( do
not use student names)
5.
Evidence of use of technology
6.
Evidence of leadership in or out of education
7.
Evidence of involvement in school improvement initiatives (SACS,
or SIP, or other)
You may choose to submit this supporting
material in Vita or Resume format and
it will be submitted in the FOED 6020 class.
Additional
Requirements
Candidates for the MTSU Leadership
Program will also make application to the Graduate College at Middle Tennessee
State University. Candidates will be
expected to supply official transcripts and other qualifying materials to the
Graduate College. Admission to the
Graduate College is separate from being accepted to the Leadership
Program. For your convenience, Graduate
School applications and fees will be collected during the initial class
meeting. (There is a $25 application fee
to apply to the MTSU Graduate College.)
