Graduation Gown Order Form for May 2012

The last day to submit an order is April 10, 2012!

This order form is only for ordering a gown for graduation. If you will be receiving a master's or doctor's hood, honor stole or honor cords, you will receive them at the time you pick up your gown. We will have that information the Friday before gowns are given out on Monday.

Please make sure all information is completed on your order form (name, email, degree, height/weight information). Orders with incomplete information will not be ordered.

Please print a copy of this order before you submit it to the bookstore. Regalia pick-up dates for May 2012 graduation are April 30 - May 4 at Phillips Bookstore in the Keathley University Center during the following times: Monday - Thursday 8am-6pm and Friday 8am-3pm.

Regalia will not be mailed or shipped to graduates. You must make arrangements to pick up regalia at Phillips Bookstore during the above times.
