Graduation Gown Order Form for December 2009

Please complete the following form at least 1 month prior to the December 2009 graduation ceremony.

This order form is only for ordering a gown for graduation. If you will be receiving a honor stole or honor cords, you will receive the stole or cords at the time you pick up your gown. We will have that information the Friday before gowns are given out on Monday.

*All caps are one-size with an elastic band.

Please make sure all information is completed on your order form (name, email, degree, height/weight information). Orders with incomplete information will not be ordered.

Please print a copy of this order before you submit it to the bookstore. Regalia pick-up dates for December 2009 graduation are December 14 - 18 at Phillips Bookstore in the Keathley University Center during regular store hours.

Regalia will not be mailed or shipped to graduates. You must make arrangements to pick up regalia at Phillips Bookstore during the above times.
